Pes growth are gotten from 2 methods

1. Birth rate, more pes = more births. Affected by med science.

2. Empty houses and jobs available, peoples move in to work. Not affected by med science

So medicine science will give good peasants growth in only the mid or late stage where your total pes count is high and not in the early era.
